What am I supposed to put in the "Born to" field? The person's Mother and/or Father?

Should I use both symbols at the same time?

And, would it be safe to assume that if one does not know the name of the Father or Mother or Date of Birth, that the treatments should not be executed?

Sure, you can use both n.2 symbols at the same time (Om Symbol+Salomon Symbol or two universals symbols of your radiesthesic choice)

for an correct and guaranteed radionic emission, its better to introduce in the software boxes:

Name+Surname+date of Birth+hour of birth+address of residence
